Don Athletes & Sports Clearance Day

All student-athletes (all sports) must have a physical dated AFTER June 1, 2023 to participate in sports for the entire school year.  Acalanes Boosters will be hosting its annual fundraiser that allows student-athletes to be seen by a physician in order to be cleared to participate in Acalanes Athletics.  This year they have teamed up with Golden State Orthopedics, (which has our very own Acalanes doctors Mike Michlitsch and Matt Pecci) to perform a physical for a cost of $50.  All proceeds are donated back to Acalanes Boosters, and we handle all of the paperwork…it’s a win-win!

The Athletic Clearance fundraiser will be held on SATURDAY, JULY 15TH 9am to 1pm at 2625 Shadelands Dr. in Walnut Creek.    In the past, this has been done on campus at Acalanes but we holding the event at Golden State Orthopedic to provide more privacy for our student-athletes and allow our doctors to perform a more thorough exam using their equipment in their offices.

Here is what you need to do:

1.  Make an appointment and pay to secure your spot.  NO WALK-INS allowed.  Use this link:

2.  Register your player on SportsNet:  Click Here

3.  Print out these forms.  Fill out HISTORY form only, and bring all forms to the exam.  Make sure a parent’s signature is on it: Athletic Clearance Medical Forms

4.  After the player sees a physician, we will handle the paperwork.  Nothing more to do!

If you are unable to have your player see a physician on this date, your player will need to have a physical done prior to the start of your sport season. For Fall sports that would be Monday, August 7th. You will still register your player on the SportsNet site and you will need the forms from the link above to bring to your doctor.  You will also have to upload the forms on the SportsNet site once your player has been cleared to play
